บิดา มารดา คู่สมรส และบุตร เสียชีวิต

การขอรับสวัสดิการกรณีบิดา มารดา คู่สมรส บุตร (ยังไม่บรรลุนิติภาวะ) ของสมาชิกเสียชีวิต

สมาชิกต้องยื่นขอรับสวัสดิการภายใน 1 ปี นับตั้งแต่วันที่เสียชีวิต ทางสหกรณ์จ่ายเงินช่วยเหลือโดยคำนวณตามอายุการเป็นสมาชิกถึง ณ วันที่เสียชีวิต ดังนี้

อายุการเป็นสมาชิก เงินสวัสดิการที่ได้รับ
ไม่เกิน 10 ปี 10,000  บาท
10 ปีขึ้นไปแต่ไม่เกิน 15 ปี 20,000 บาท
15 ปีขึ้นไป 30,000  บาท

หลักฐานประกอบ

  1. แบบฟอร์มขอรับสวัสดิการกรณีบิดา มารดา คู่สมรส บุตร(ยังไม่บรรลุนิติภาวะ) เสียชีวิต ดาวน์โหลดแบบฟอร์ม
  2. เอกสารของผู้เสียชีวิต (สมาชิกรับรองเอกสารทุกฉบับ)
  • สำเนาใบมรณบัตร จำนวน 1 ฉบับ (กรณีไม่ได้นำต้นฉบับมาให้ผู้บังคับบัญชารับรองเอกสาร)
  • สำเนาบัตรประชาชน 1 ฉบับ
  • สำเนาทะเบียนบ้าน ประทับตรา “ตาย” ของผู้เสียชีวิต จำนวน 1 ฉบับ
  • สำเนาใบสำคัญการเปลี่ยนชื่อตัว และหรือ ชื่อสกุล(ถ้ามี) อย่างละ 1 ฉบับ
  1. เอกสารของทายาทผู้รับผลประโยชน์
  • สำเนาบัตรประชาชนคนละ 1 ฉบับ
  • สำเนาทะเบียนบ้านคนละ 1 ฉบับ
  • สำเนาทะเบียนสมรส (กรณีทายาทป็นคู่สมรส) 1 ฉบับ
  • สำเนาใบสำคัญการเปลี่ยนชื่อตัว และหรือ ชื่อสกุล(ถ้ามี) อย่างละ 1 ฉบับ

วิธีการยื่นขอรับสวัสดิการ

  1. ให้สมาชิกยื่นแบบฟอร์มพร้อมแนบหลักฐานประกอบด้วยตนเองที่สหกรณ์ หรือจัดส่งเอกสารทางไปรษณีย์
  2. เมื่อเจ้าหน้าที่ตรวจสอบเอกสารครบถ้วนถูกต้อง จะดำเนินการตามขั้นตอนเสนอผู้มีอำนาจลงนามอนุมัติ
  3. เมื่อผู้มีอำนาจลงนามอนุมัติแล้ว ทางสหกรณ์จะใช้ระยะเวลาประมาณ 7-14 วัน ในการโอนเงินสวัสดิการให้กับสมาชิกต่อไป
